Opportunity to get in on ground-breaking virtual care Advanced Practice Provider work!

Virtual Care, or to many known as Telehealth, continues to grow in utilization and importance in health care delivery.  The IU Health Virtual Care team, in partnership with IU Health Physicians Community Medicine, seeks an Advanced Practice Provider with primary care experience.  The position will be part of a dynamic team of centralized clinicians and administrators providing care across multiple specialties including psychiatry, behavioral health, palliative, and hospital care. The selected individual will build a virtual care program to deliver virtual visits within the IUH network for at-risk patients, including on-demand low-acuity visits, Medicare Wellness visits, & general initial and follow-up care. Medicare Wellness visits will be done alongside the “smart hands” team in home, with the APP providing care virtually. The role will involve guiding protocols and guidelines development, sharing the work with hospitals, providers and patients. Innovative thinking and designing new pathways for care are central to role. Work hours will be primarily days; primary location will be downtown Indianapolis, from where virtual-care delivery originates. The role will be a blend of administrative (speaking with physicians, hospitals and clinics, guiding program development including protocols and guides) and clinical time. Growth opportunities will exist within leadership and program development. APP will work in innovative implementation of program to assess/evaluate patients presenting from home, collaborating with in-home clinician to assess and evaluate patient presentation and current health status via secure video interface.  Follow-up communication will occur with primary care provider (if appropriate). Documentation of current patient health status to fulfill required annual visit. Potential exists for on demand virtual encounters for low acuity illness with patients seeking online ‘walk in clinic’ type care delivery.
